The letter of support can be brief. Please be sure to include: - About your organization (how long you've been around, what services you provide and where). - If you are a member of Inclusive Alliance, how long you have been a member. - Why you support Inclusive Alliance's SCN (ideas include: our commitment to collaboration, years of experience, reputation in the community). - Contact information for your organization. ###### Who should I contact if I have questions? We're here to support your needs. Please reach out to [info@inclusivealliance.org](mailto:info@inclusivealliance.org) if you have any questions or need additional support. ###### Who can apply to be a SCN Lead? 501c3 nonprofits, including CBOs, IPAs, Health Homes, BHCCs, FQHCs, or PPSs with at least 3 years of experience working with CBOs in the application region. ###### Why Support Inclusive Alliance? Inclusive Alliance is aspiring to lead the way in developing a robust social care network for Central New York & Mohawk Valley Region 7, and we believe that the best way to do this is collaboratively.  We exceed all of the criteria required to be the lead SCN entity.  Inclusive Alliance is a 501c3 nonprofit IPA, formed in 2016. Our network is made up of nearly 50 Community Based Organizations spanning the CNY region including Cortland, Herkimer, Madison, Oneida, Onondaga and Oswego counties. Our fully virtual team is based entirely in Central New York.
